Technology is helping doctors reach those most in need while reducing hospital admissions, according to a local charity.
Family Nursing & Home Care (FNHC) has launched a two-year project, the Graphnet Remote Monitoring Pilot, designed to support patients to live safely and independently at home in Jersey.
Using digital healthcare platform Luscii, patients can submit their health data remotely. This information is monitored by healthcare professionals, who can respond quickly and visit patients if any concerns are identified.
Kathryn Kelly from FNHC said the project had already “made a difference”, highlighting how clinical data has enabled early intervention.
The first year of the pilot will focus on frailty, before expanding to diabetes and wound care in year two.
The initiative has been funded through Impact Jersey’s Care Tech Challenge, which supports the development of innovative health technology solutions.
Dr Chris Edmond, Clinical Lead for Impact Jersey, said the programme aims to help people “live their best lives for as long as possible”, while also easing pressure on Jersey’s healthcare system.
He added that, while technology should not replace the human aspect of care, it can support more efficient and targeted healthcare delivery.
